2026 Benefits – Key Changes

Keystone HMO:

Deductible increase $2,000 (single) / $6,000 (family) on certain services where deductible applies.

Keystone POS:

Co-insurance increase to 20% on certain services where deductible applies.

Personal Choice PPO:

Copay increases:

  • Primary Care Physician (PCP): $20
  • Specialist/Therapy/Diagnostic: $40
  • Outpatient Surgery: $200

High-Deductible Health Plan (HDHP):

There will be no charge / no deductible for Virtual Care services through Teladoc.

Employer HSA Contributions:

  • PHMC will continue to make HSA contribution deposits on a per pay basis for employees enrolled in the HSA Qualified HDHP. The annual PHMC contribution will total $700 for all coverage tiers.
  • 2026 HSA Annual Maximums
    • $4,400 – single
    • $8,750 – family
  • $1,000 catch-up contribution for individuals 55 and older.

FSA – 2026 Maximums:

  • Heath Care – $3,400/annual
  • Dependent Care – *$7,500/annual
  • Parking – $340/monthly
  • Transit – $340/monthly

*Dependent Care FSA Limit – The IRS increased the dependent care FSA annual limit for 2026 to $7,500 to reflect inflation and implement provisions from the One Big Beautiful Bill (OBBB). Employers are not required to adopt the new limit; however, PHMC has chosen to adopt the new limit increase of $7,500 annually for dependent care accounts. This is the first increase to the dependent care limit since 1986.
