Sarah Hexem Hubbard, Esq. oversees PHMC’s Community Health Services portfolio, which includes PHMC Health, Health Promotion Services, and two PHMC subsidiaries: Health Promotion Council and the National Nurse-Led Care Consortium (NNCC). In this role, Sarah is responsible for interdepartmental collaboration to align PHMC services that impact people’s primary care and wellness needs. Within this portfolio, she serves as NNCC’s Executive Director, working at the national, state and local levels to strengthen community health through collaborative, compassionate and high-quality nurse-led models of healthcare.

Sarah applies her background in law and policy to advocate for nurses practicing in community-based settings and, in turn, nurses advocating for the communities they serve. Within NNCC, she also leads the Pennsylvania Nursing Workforce Coalition, a statewide collaborative that advances a nursing workforce that will lead health care transformation by cultivating strategic partnerships with diverse populations and organizations. 

Sarah regularly speaks on topics related to the role of nursing in building a culture of health. She has published in the areas of public health law research, nurse practitioner scope of practice, and medical-legal partnerships. Sarah teaches health policy at the University of Pennsylvania School of Nursing, and she graduated magna cum laude from Temple Law School.
