Supportive Housing Services

PHMC’s continuum of care for people experiencing homelessness includes health, social services, supportive services, and case management at shelters and with formerly homeless adults who are living in transitional and permanent housing.

Note PHMC cannot accept direct referrals for housing programs. To connect with housing, please refer to this list of shelters and facilities.

Emergency Housing programs provide case management for families and single women in an Emergency Housing (Shelter) setting. Services include housing assessments, linkages to supportive services, connections to mainstream benefits, and more.

Transitional Housing programs provide case management for individuals and families experiencing temporary disruptions in housing.

Rapid Rehousing provides short-term (1-3 months), and medium-term (4-24 months) rental assistance and case management/housing stabilization services. The goals of the Rapid Rehousing program are to help households experiencing homelessness obtain housing quickly, increase self- sufficiency, and reman stably housed after the rental assistance ends.

Permanent Housing programs build self-reliance and independence by providing social services to help individuals and families navigate through housing, financial, and health care concerns while living in HELP Philadelphia and/or West Philadelphia Real Estate (WPRE) housing.

HOUSING SUCCESS STORY: TENANT-BASED RENTAL ASSISTANCE PROGRAM

Tony* spent ten years on the waitlist for a Housing Choice Voucher from the Philadelphia Housing Authority. During part of that period, he lived with his mother, but when she relocated out of state, Tony experienced homelessness, primarily couch surfing between friends and family. His health had deteriorated to a critical point when he connected with PHMC’s Tenant-Based Rental Assistance (TBRA) program, which provides permanent supportive housing to individuals diagnosed with HIV and/or AIDS.

With the support of the TBRA program, part of PHMC’s division of Housing & Justice-Related Services, Tony transitioned into an affordable one-bedroom apartment. Tony took advantage of the program’s connections to health management and financial wellness services, and both his health and housing stability improved. His landlord has never reported any concerns with Tony’s tenancy, and once Tony received his Housing Choice Voucher subsidy, his landlord gladly accepted the voucher.
