PHMC Health delivers high-quality, nurse-led primary care through five Federally Qualified Health Centers across Philadelphia. Each is recognized as a Patient-Centered Medical Home, and collectively they serve 20,000 adults and children annually, including people experiencing homelessness, residents of public housing, LGBTQ+ individuals and adults with chronic conditions. Our model integrates prevention, behavioral health, social services, dentistry and pharmacy services.
Beyond our health centers, we have nurse-home visiting programs. PHMC also partners with the Office of Homeless Services (OHS) to provide primary care through on-site nursing in fifteen city shelters. We also operate two medical respite programs that provide post-acute care and safe, supportive environments for recovery to individuals transitioning from hospitalization to shelters or stable housing.
