PHMC offers a range of services that help families access the resources and support they need to thrive. These include care coordination, home visiting, health education and assistance navigating medical, developmental and behavioral health systems. Families can also receive support with child care access, provider referrals and financial assistance for early learning. Whether connecting families to critical services or guiding them through complex systems, PHMC’s work is rooted in meeting people where they are and strengthening long-term well-being.
