Our programs support the health, stability, safety and development of children and families through a wide range of services that span early childhood, parenting, foster care and social services. From referring families to high-quality child care to supporting parents through home visiting and fatherhood initiatives, our work utilizes strong partnerships and a preventive approach.

Parent & Child Programs

PHMC’s Parent & Child Programs are designed to support caregivers and promote healthy child development from birth through early childhood. Through home visiting, parenting education, fatherhood support and early literacy, these programs strengthen family relationships and help children build strong foundations for learning and life.

PHMC offers a range of services that help families access the resources and support they need to thrive. These include care coordination, home visiting, health education and assistance navigating medical, developmental and behavioral health systems.

Foster Care

PHMC supports children and families through a continuum of foster care services focused on safety, stability and long-term well-being. In partnership with public agencies and community providers, we work to prevent out-of-home placements, reunify families whenever possible and ensure permanent, nurturing homes for children in care.
