Health Promotion Council (HPC) operates Umbrella Hub Organizations (UHOs) in Pennsylvania and Delaware. These UHOs centralize data collection, claims processing and reimbursement for National Diabetes Prevention Program (DPP) providers. HPC is a subsidiary of PHMC.
- Managing data submission to the CDC for program recognition
- Processing claims and distributing reimbursements to partner organizations
- Supporting partners in securing Medicaid & Medicare contracts
- Expanding DPP provider networks in PA & DE
- Direct delivery of National DPP services by HPC

Eligibility Criteria
The Prediabetes Risk Test and Make A Choice website are open to all
Program eligibility includes:
- Adults age 18+
- Diagnosis of prediabetes and/or A1C levels of 5.7%-6.54%, at high-risk for developing prediabetes (receiving a score of 5 or higher on the Prediabetes Risk Test)
Referral Process
Participants may access programming and information directly. Participants may also be connected through any source, including health care or insurance providers.
