The Technology Assisted Children’s Health Program (TACHP) provides home visiting services for children ages 0-22 with special health care needs requiring medical technology. The Technology Assisted Children’s Program delivers personalized care and support to empower families in managing their children’s unique needs. TACHP is a program of Health Promotion Council (HPC), a PHMC subsidiary.

Eligibility Criteria

Child/youth who meet the following:

  • Age 0-22 years
  • Live within one of the specified eastern counties in Pennsylvania

“Technology-assisted” refers to the use of a medical device to compensate for the loss or diminished capacity of a vital body function.

  • Medical devices include: feeding tubes, colostomy bags, catheters, EKG monitor and pacemakers, ventilators or cardiac shunts.

Referral Process

Participants can enroll directly.
