Supportive Housing Services
PHMC offers a comprehensive continuum of care designed to support individuals and families experiencing or at risk of homelessness. Services integrate health, social services and case management to address immediate needs and promote long-term stability. Through these comprehensive programs and services, PHMC strives to empower individuals and families, helping them achieve stability and self-sufficiency.

Resident of a permanent housing facility that has contracted with PHMC to provide supportive case management services
Referral Process
Supportive Housing Services programs are not open to the public. Participants access services through the PHMC-contracted facility where they reside, or through a referral from an agency such as the Office of Homeless Services.
