Supplemental Nutrition Assistance Program Education (SNAP-Ed)
The Supplemental Nutrition Assistance Program Education (SNAP-Ed) empowers individuals and communities with the knowledge and resources to make healthier food choices and adopt active lifestyles. As an implementing partner, HPC plays a vital role in delivering nutrition and wellness education while expanding access to nutritious food options. Through strategic partnerships, these services are delivered at schools, pantries, shelters, grocery stores, transitional housing, and community centers, targeting factors at the individual, interpersonal, community, policy, and societal levels. - Policy, System, and Environmental (PSE) changes to improve community health

SNAP eligibility is determined mainly by income and household size; for a household of four, gross monthly income cannot exceed $5,200.
Referral Process
The SNAP website is available to all.
SNAP programming is delivered to SNAP-eligible individuals and families who may already receive SNAP benefits.
