Sickle Cell Community Services
Sickle Cell Community Services (SCCS) supports individuals and their families affected by Sickle Cell Disease (SCD), through coordinated care across systems. SCCS’s focus on coordinated holistic care not only strengthens communication between clients, families, and care teams; it also improves collaboration across public health and service systems.
In addition to its work at the individual level, SCCS aims to increase community awareness and understanding of SCD by offering education about the disease and promoting equitable access to supportive services. Sickle Cell Community Services is a program of PHMC subsidiary Health Promotion Council.
- Help improve health outcomes by assisting clients to integrate fully into their communities and access all available supports
- Support lifelong transition planning from childhood through adulthood
- Connect clients to education and employment resources
- Build peer, community, and family support networks
- Provide education regarding SCD and Cell and Gene Therapy (CGT) to individuals, partner organizations, and communities

Eligibility Criteria
- Children, teens, and young adults up to age 21
- Adults age 22 and older, in some cases (contact us for more information on eligibility)
Live in the Lehigh or Capital regions, including the following counties: Adams, Berks, Cumberland, Dauphin, Franklin, Fulton, Huntingdon, Lancaster, Lebanon, Lehigh, Northampton, Perry, York
Referral Process
Participants can enroll directly or may be connected through an agency or organization.
