PHMC Health’s shelter nurses provide primary care services in nearly 20 family shelters and support street outreach teams for people experiencing homelessness in Philadelphia. In partnership with the Office of Homeless Services, PHMC Health provides primary care, well checks and sick visits to families directly at the shelters. These services reduce unnecessary hospital visits and improve health outcomes for people experiencing homelessness.

  • Adult and pediatric primary care
  • Immunizations
  • Management of the Healthy Babies Initiative, which includes assessment, monitoring and support services to infants up to four months of age and their mothers in shelters
  • Case management support to connect patients with social services

Eligibility Criteria

Adults and children

Currently live in an Office of Homeless Services (OHS) designated shelter

Referral Process

Patients must access services through the OHS-designated shelter where they reside.
