Postpartum Visit Program

The Postpartum Visit Program connects a maternal-child health nurse to individuals who have recently given birth to ensure everything is going well. A nurse conducts a health assessment of the postpartum parent and baby while providing connections to additional resources. The Postpartum Visit Program is part of the National Nurse-Led Care Consortium (NNCC), a subsidiary of PHMC.

Eligibility Criteria
- Must be a Health Partners Plan Medicaid member
- Between 7 and 84 days after delivery of a child
- Identified by Health Partners Plan as in need of a postpartum follow-up visit and care
Referral Process
Patients must access services through their Health Partners Plan.
