Philly-MontCo Family Center operates a Parents as Teachers model for personal home visiting, servicing families with children from prenatal to kindergarten. This evidence-based model aims to increase parent knowledge of early childhood development and improve positive parenting practices. Philly-MontCo Family Center is a program of National Nurse-Led Care Consortium (NNCC), a PHMC subsidiary.

Eligibility Criteria

Families are eligible to enroll prenatally to six months before the youngest child starts Kindergarten.

Caregiver and child(ren) must be available for 60-minute visits twice a month between 8:30 a.m. and 5:30 p.m.

  • Live in Philadelphia
  • Household income must be below 250% of the federal poverty level

Referral Process

Families can enroll directly or may be connected through an agency such as the Office of Children and Youth, Philly Families CAN, or Keystone.
