The Philadelphia Nurse-Family Partnership (NFP) is an evidence-based community health program for first-time pregnant people. Home visitors who are Registered Nurses meet with clients to provide individualized support.

By helping to break the cycle of poverty, the Nurse-Family Partnership plays an important role in helping to improve the lives of society’s most vulnerable members, build stronger communities and leave a positive impact on this and future generations. NFP is a program of National Nurse-Led Care Consortium, a PHMC subsidiary.

  • Specialized support and information that meets clients where they are

  • Education and support on nutrition, lactation, child development, mental health and more

Eligibility Criteria

Pregnant with a first baby (NNCC encourages families to enroll before 27 weeks, but can enroll a limited number of clients past 28 weeks)

  • Live in Philadelphia
  • Household income must be below 250% of the federal poverty level

Referral Process

Participants can enroll directly or may be connected through a provider such as Philly Families CAN.
