Opioid Linkage to Care
The Opioid Linkage to Care program provides support, promotes harm reduction and delivers prevention services for individuals with opioid use disorder. This program reaches out to high-risk communities, engages individuals with opioid use and evaluates their needs for harm reduction interventions, along with addressing other social determinants of health.
Part of PHMC program New Pathways Project (NPP), Opioid Linkage to Care helps connect participants to treatment readiness services at NPP or other community-based licensed treatment providers and primary care services. It also offers education and engagement for families and community members in opioid overdose prevention and intervention.

Eligibility Criteria
Adults 18+ in communities with high risk for opioid overdose in Philadelphia
No insurance required
Referral Process
Participants may enroll directly or connect through an agency such as a treatment provider or community-based organization.
Community-based organizations are welcome and encouraged to refer participants.
