National Diabetes Prevention Program (National DPP)
The National Diabetes Prevention Program (DPP) is an evidence-based lifestyle change program designed to reduce the risk of developing type 2 diabetes. Over 22-26 sessions, it supports individuals with prediabetes or high-risk factors through structured coaching, physical activity and weight management strategies over 12 months. The National DPP is offered by the Health Promotion Council (HPC), a PHMC subsidiary.
- Sixteen weekly one-hour sessions led by trained lifestyle coaches
- Bi-monthly one-hour follow-up sessions for six months
- Guidance on achieving 150 minutes of physical activity per week
- Support for reducing calorie intake and achieving 5-10% weight loss
- Delivery options: online (asynchronous), in person or live virtual sessions
- Insurance reimbursement through Medicare, Medicaid (PA & DE) and private insurers

Eligibility Criteria
Adults 18+
Diagnosis of prediabetes and/or A1C levels of 5.7%-6.54%, at high-risk for developing prediabetes (receiving a score of 5 or higher on the Prediabetes Risk Test)
No previous diagnosis of type 1 or type 2 diabetes
