Make a Choice – Chronic Disease Self-Management Programs

Make a Choice provides health self-management programs that offer a choice of free education and support to help individuals manage chronic disease, chronic pain, arthritis and diabetes. Delivered over six weeks by trained facilitators, sessions are available in person, online or by phone in small group settings. Make a Choice is a program of Health Promotion Council (HPC), a PHMC subsidiary.
- Weekly goal-setting and peer support
- Chronic Disease and Diabetes Self-Management programs
- Arthritis Foundation’s Walk With Ease Program (English & Spanish)
- Virtual, in-person and phone-based options
- Proven positive health outcomes and reduced emergency visits

Eligibility Criteria
Adults age 18+
Anyone living with chronic disease/diabetes/chronic pain
Further eligibility varies by program
Referral Process
Anyone is welcome to participate.
Additionally, referrals may come from:
- Program service delivery organizations
- Health care providers
- Community-based organizations
