LifeSet provides young adults aging out of the foster care system with the necessary skills and resources to live successfully at this critical juncture in their young lives. The program model integrates both clinical and case management; LifeSet staff is available 24/7, with face-to-face sessions once per week or more to match the individual needs of each young adult.
Lifeset is a program of Turning Points for Children, a PHMC subsidary.

Eligibility Criteria
Young adults age 17-23
Leaving foster care, juvenile justice or mental health systems
Referral Process
Youth can enroll directly, or they can be connected by an agency or caring loved one.
