Family-Based Services

Family-Based Services are an intensive continuum of family therapy, case management and other supportive services delivered directly in the family’s home. The program aims to prevent out-of-home placement of children and adolescents with serious emotional issues. Staff specialize in providing services to families impacted by trauma, families managing ongoing medical issues and Spanish-speaking families.

Eligibility Criteria
Youth must have a prior behavioral health diagnosis and a psychiatric evaluation recommending Family-Based Services
Both youth and caregiver(s) must be willing to actively participate in several hours of in-home family therapy per week
Live in Philadelphia
Referral Process
- Participants will need a referral from the City of Philadelphia’s Community Behavioral Health (CBH) program.
- Program staff is available to support on getting a referral.
