Empowering Asthma Self-Management (EASE) Allentown

EASE Allentown is an 18-month initiative in Allentown to support older adults living with asthma. In collaboration with the Allentown Health Bureau and funded by the Asthma and Allergy Foundation of America, the program provides self-management education and resources to help individuals reduce asthma triggers and improve their quality of life. The EASE Allentown program is offered by Health Promotion Council (HPC), a PHMC subsidiary.

Eligibility Criteria
Older adults (primarily age 50+)
Live with asthma
Live in Allentown, PA
Referral Process
- Participants access services through Allentown Health Bureau home visiting programs and local health care providers.
- Participants are identified through community asthma screenings and health assessments facilitated by local health services, as well as through a public awareness campaign.
