The Community to Home (C2H) program provides home visiting services for children and youth with special health care needs in south central and northeastern rural Pennsylvania. Using an evidence-based Community Health Worker model, C2H reduces barriers, promotes equitable health care access and creates continuous care plans for children with newly-diagnosed or potential special health care needs, fostering wellness through trusted community relationships. C2H is a program of Health Promotion Council (HPC), a PHMC subsidiary.

Eligibility Criteria

Children age 21 years and under

Newly diagnosed with special health care needs OR at risk of a diagnosis OR newly residing in a rural Pennsylvania county

Households with an income of 300% or less than the federal poverty level

Referral Process

Participants can enroll directly or may be connected through an agency or organization.
