PHMC is hiring service coordinators and support coordinators to ensure that services are provided to individuals and families in Philadelphia and surrounding counties. ChildLink, a program of PHMC, provides supports and service coordination to children ages birth to three years old and their families who reside in Philadelphia and have developmental delays or disabilities. PersonLink, a program of PHMC, provides supports services to individuals diagnosed with intellectual disabilities.

We are looking for candidates who have:
- a Bachelor’s degree in the social sciences, human services or education.
- at least one year of experience working with or providing services to children, families or individuals with disabilities – ideally in home and community settings.
- the ability to speak and write in Spanish or other second languages (incl. ASL) – preferred but not required
