PHMC continues to raise awareness of the affordable and quality care provided at its five nurse-managed, federally qualified health centers while introducing new initiatives to provide “wrap around” care to the regions underserved and homeless populations. In recognition of this expanding role, the theme for this year’s National Health Center Week is “Celebrating Our Legacy, Shaping Our Future.”

In Southeastern Pennsylvania, more than 198,000 adults use a community health center or clinic as their source of regular, primary care and 20 percent of uninsured adults in the region are more likely to use public health centers, despite the availability of coverage under the Affordable Care Act. This data comes from the 2014/2015 Southeastern Pennsylvania Household Health Survey, conducted by the Community Health Data Base, a PHMC resource.

  • The Rising Sun Health Center broke new ground as one of the first nurse-managed FQHCs in the nation to offer integrated legal and financial services in partnership with Community Legal Services (CLS). Clarifi, a PHMC affiliate provides the financial counselling.
  • PHMC and Coordinated Care Network (CCN) partnered to open a pharmacy at the PHMC Care Clinic, a nurse-managed FQHC located at 1200 Callowhill Street.
  • In March, the PHMC Care Clinic introduced a Diabetes Academy. This program helps support the Clinic’s patients who have diabetes by teaching them about the disease, the importance of taking their medication, diet and exercise.
  • PHMC Health Connection’s partnership with PHMC affiliate, Joseph J. Peters Institute (JJPI), offers on-site assessments and counseling for individuals who have experienced sexual abuse or other forms of trauma.
  • The Homeless Health Record was created for PHMC’s Healthcare for the Homeless Program, which provides health care, social services and behavioral health care to Philadelphia’s homeless population. The health record is web-based and available via a mobile application for providers.
  • Last year, as the Healthcare for the Homeless designee in Philadelphia, PHMC worked with Depaul USA to launch a first-of-its-kind homeless medical respite center pilot program for homeless individuals in Philadelphia.
