Eligibility Criteria
Low- to moderate- income individuals living with HIV
Live in Bucks, Chester, Delaware, Montgomery and Philadelphia Counties in PA; and Burlington, Camden, Gloucester, and Salem Counties in NJ
Referral Process
HIV-positive individuals can enroll directly.
Participants may also connect through a referral from a medical case manager.
