The Medical Home Community Team (MHCT) program provides intensive, evidence-based, homegrown and high-quality home visiting services. Programs support social services and health care coordination and advocacy for children ages 0-21 with or without special healthcare needs. These programs are offered by Health Promotion Council (HPC), a PHMC subsidiary.

Eligibility Criteria
Families with children and youth from birth to age 21
Insurance-eligible only
Referral Process
Families will need a referral from a contracted Managed Care Organization.
