The Health Referral Hub is a centralized resource navigation service available statewide in Pennsylvania. It connects individuals to disease self-management and prevention programs, as well as resources for health-related social needs identified through Social Determinants of Health screenings. This hub is offered by Health Promotion Council (HPC), a PHMC subsidiary.
