The Mabel Morris Family Home Visit Program is a voluntary, no-cost program for Philadelphia families with children ages five and under. Registered nurses partner with caregivers to understand and celebrate the strengths and goals they have for their children. They suggest activities, strategies, resources and encouragement to match. This collaboration builds on families’ existing abilities to nurture school-ready, lifetime learners. Mabel Morris is a program of National Nurse-Led Care Consortium (NNCC), a PHMC subsidiary.

Eligibility Criteria
Families are eligible to enroll prenatally to six months before the youngest child starts Kindergarten.
Caregiver and child(ren) must be available for 60-minute visits twice a month between 8:30 a.m. and 5:30 p.m.
- Live in Philadelphia
- Household income must be below 250% of the federal poverty level
Referral Process
- Families can enroll directly or may be connected through a provider.
- Please note there is currently a six-month waiting period.
