The Keys to Quality Health Center Controlled Network (K2Q HCCN) is a network of participating health centers working together to leverage health information technology and data to enhance how they deliver affordable, accessible and high-quality primary care. K2Q HCCN partners connect health centers to resources, provide individualized and peer training and support, and facilitate health center collaboration. PHMC is the grantee on record for K2Q HCCN, a program of PHMC subsidiary National Nurse-Led Care Consortium (NNCC).
- Customizable workplans: individualized assessments and workplans tailored to their strategic priorities, HIT capabilities and operational needs
- Comprehensive support: one-on-one technical assistance, access to expert consultants and peer learning opportunities
- Nurse-Led Expertise: specialized support for team-based care delivery, workforce development and community-led public health initiatives
- Non-Medical Health Factors Integration: tools to assess and address non-medical health factors through enhanced care coordination and data-driven strategies to improve health outcomes
