Healthy Heart Ambassador Blood Pressure Self-Management
The Healthy Heart Ambassador Blood Pressure Self-Management program is a one-year initiative, in partnership with Enon Tabernacle Baptist Church, that supports 25 members living with high blood pressure. Two trained staff deliver the Healthy Heart Ambassador Blood Pressure Self Management program, providing participants with blood pressure monitors and four months of follow-up to ensure proper monitoring techniques and communication with health care providers.
- Blood pressure self-monitoring education
- Blood pressure monitor distribution and tracking
- Guidance on communicating with primary care providers
- Four nutrition workshops promoting healthy eating and physical activity

Eligibility Criteria
Member of Enon Tabernacle Baptist Church
Adults with high blood pressure
Adults with no recent cardiac episode (heart attack) or arrhythmia
Referral Process
The program is currently full and not accepting new participants.
When the program is open:
- Participants may enroll independently or connect through church announcements, faith-based outreach, and partnerships with community health organizations.
- This program offering can be expanded to other partners with additional funding or contracted with payers, such as employers, health plans or health care systems.
