A Community Care Hub (CCH) addresses the health and social needs of community members by connecting them to vital services such as health education, support programs, and assistance with housing, food, transportation, utilities and other social determinants of health. Health Promotion Council, a PHMC subsidiary, serves as a Community Care Hub and collaborates closely with the Montgomery and Berks County Area Agencies on Aging (AAA), the Pennsylvania Association of AAAs, and an Advisory Council to deliver these essential services to the community.
- Connect community-based organizations, health and social care providers and clients to No Wrong Door systems in Pennsylvania for easier referrals
- Contract with health care providers and health plans for Community Health Integration, Principal Illness Navigation, and disease management and prevention services in its network

Live with high blood pressure
Referral Process
Participants can enroll directly or can be referred by health care providers, insurance providers, or via internal/external agencies.
