PHMC’s MyVaccineCounts initiative, funded by Health Resources and Services Administration (HRSA), has successfully concluded on February 28, 2023. The initiative addressed persistent health disparities and vaccine hesitancy in Pennsylvania and Delaware through Community Health Workers that offered support and resources to vulnerable and under-resourced communities, with a goal to identify individuals eligible to receive the COVID-19 vaccine. PHMC worked with 159 Community Health Workers across 22 partner organizations in Pennsylvania and Delaware that administered 3,966 COVID-19 vaccine doses and reached 2,071,321 people through virtual engagement and 1,421 in-person outreach events.*

MyVaccineCounts objectives included vaccine deployment, outreach, and education across Pennsylvania and Delaware through Community Health Workers; partnership with Community Based Organizations to address persistent disparities and vaccine hesitancy; and public awareness and education via a media campaign to overcome myths related to the COVID-19 vaccines.

The campaign toolkit, which is still available for public use, includes videos and graphics, educational materials, and other resources on the COVID-19 vaccines and boosters that anyone can share at events, over email, or on social media. Access the campaign toolkit here.
