PHMC is hiring service coordinators and support coordinators to ensure that services are provided to individuals and families in Philadelphia and surrounding counties. ChildLink, a program of PHMC, provides supports and service coordination to children ages birth to three years old and their families who reside in Philadelphia and who have developmental delays or disabilities. PersonLink, a program of PHMC, provides supports services to individuals diagnosed with intellectual disabilities.

We are looking for candidates who have:
- a Bachelor's degree in the social sciences, human services or education.
- at least one year of experience working with or providing services to children, families or individuals with disabilities – ideally in home and community settings.
- the ability to speak and write in Spanish or other second languages (incl. ASL) – preferred but not required
