The PHMC Health Network's five nurse-managed, Federally Qualified Health Centers play a critical role in assisting underserved communities, particularly public housing populations and individuals experiencing homelessness, by providing high-quality, patient-centered primary care including an array of services – substance use disorder treatment, behavioral health, dental, trauma, pediatric and social services. Monday, August 10: Healthcare for the Homeless Day
Collection drive: Drop off resources for our patients experiencing homelessness at any of our five locations. Items like socks, toiletries and canned goods are welcome. Call 855.887.9229 to schedule a drop off at one of our locations.

Reading Corner: We are setting up reading corners for children and adults at Woodstock family shelter.

Tuesday, August 11: Children’s Health Day
This is a reminder to get your children to complete their immunizations and well checks for back-to-school preparation. Call 855.887.9229 to schedule an appointment for your child.

We provide specialized programming for fathers through Focus on Fathers (FOF), a community-based fatherhood support program that offers parenting education, case management, and job readiness training.
