Philadelphia Health Care for the Homeless program managed by PHMC is one of the sponsors of ‘Homeless Memorial Day’, on December 17 from 5:00 -6:00p.m. to remember homeless and formerly homeless Philadelphians who lost their lives in 2015. The event will be held at 15th Street and JFK Boulevard.
PHMC’s Philadelphia Health Care for the Homeless program provides services to more than 9,000 homeless individuals and families each year which includes comprehensive primary and behavioral health services and care coordination at the Mary Howard Health Center, 12 city shelters and three winter cafés. Two PHMC nurses provide immunizations for more than 1,600 children entering the city’s shelter system. Through the Healthy Baby Initiative, nurses monitor, support and provide care coordination to new parents and infants under the age of four months in nine family shelters. PHMC operates the city’s only homeless medical respite which provides a safe and supportive place for homeless individuals to recover from illness, injury or surgery. PHMC also provides permanent and transitional supportive housing and services to more than 100 families and individuals each year.
