  • Philadelphia Education Fund, CTI Postsecondary Project ($68,500)
  • PHMC Public Health Institute, Household Health Survey for Delaware Public Health Institute ($75,000)

The winners of Innovations Fund grants to support specific PHMC efforts include:

  • PHMC and its affiliate National Nursing Centers Consortium, Get Covered Connector ($25,000)
  • PHMC affiliate Health Promotion Council, Statewide Alliance for Community/Clinical Partnership ($25,000)
  • PHMC's Research and Evaluation Group, Worksite Wellness Evaluation ($23,538)
  • PHMC program WestHaven, Recovery Through Music ($20,000)
  • PHMC's Community Based Services, PHMC Applied ($19,500)
